Report a bug or suggest an improvement β every report goes straight to the developer, with follow-up and comments.
Reporter is the direct channel for flagging a bug or proposing an improvement anywhere on the platform. Any logged-in user can report; the development team (entity #1) reviews, comments, and tracks each one from a dedicated panel.
App, file and form can be pre-filled from the URL β any page can link here with context already loaded.
Every new report fires an email to the developer and an internal notification to entity #1's team.
Staff follows up with internal comments and moves status from New to In Review to Resolved.
index.php is a single-screen form. You start by choosing the type:

The context fields β App/Module, File/Page, Form/Component β are optional but help a lot in tracking the problem down quickly. You can also check "Notify me by email when this is resolved" β the preference is saved on your report for when that feature is wired up.
Below the form, the My Past Reports table shows everything you've submitted with its current status, so you don't have to ask.
With starting status "New", your user, domain, IP and user-agent β giving technical context in case the issue needs reproducing.
Automatically sent to the staff address configured in the system, with every field from the report. If the email fails for any reason, the report is still saved β nothing is lost.
The whole team of the entity that handles reports also gets a notice inside the platform, with a direct link to the moderation panel.
